چکیده

In this study, we review the results of Menière's disease treatment using microdose gentamicin delivered directly to the round window using the Round Window Microcatheter (IntraEar, Inc., Denver, CO). A total of 18 patients were studied, with follow-up ranging from 6 to 18 months. In 15 of 18 patients (83%), tinnitus was improved significantly throughout the follow-up period. Vertigo was eliminated in all patients, and pressure was relieved in 17 of 18 (94%). Ihese preliminary data suggest that Round Window Microcatheter-delivered gentamicin is a safe and effective treatment for the reduction of tinnitus, vertigo, and pressure associated with Menière's disease.

Intratympanic Therapies for Menière's disease.

There are multiple treatment options to consider when managing patients with Menière's disease. When conservative measures fail to control symptoms of Menière's disease, escalation of interventions may be required. Targeted drug delivery to the round window with intratympanic injections allows for local application of high concentrations of medications, largely avoiding systemic side effects. I...

Intratympanic gentamicin treatment: preliminary results in two patients with Menière's disease.

Menière's disease is a vestibular disturbance characterized by episodic vertigo, tinnitus, and fluctuant hearing loss. The long-term effectiveness of available medical and nonablative surgical treatments has been variable, with frequent symptom recurrence. Vestibular neurectomy and labyrinthectomy, surgically ablative techniques, provide more permanent relief from vertiginous attacks. However, ...

Intratympanic and Round-Window Drug Therapy: Effect on Cochlear Tinnitus.

Tinnitus is a common symptom for which few existing therapeutic approaches can produce reliable reduction or elimination. Chemical perfusion involves the delivery of medication directly into the inner ear via the round-window membrane. This report discusses the use of dexamethasone or gentamicin in 20 individuals who had inner ear diseases in which disturbing cochlear tinnitus was one of the sy...

Menière's disease and gentamicin: preliminary results using the minimum effective dose and integrated therapy.

Treatment of Menière's disease is aimed at restoring a normal quality of life and preserving residual hearing, in view of the increasing frequency with which the contralateral ear is affected. Conventional medical treatment (diuretics + vasoactive drugs) leads to cure in a large percentage of patients (75-95%). In intractable cases, transtympanic (intratympanic) aminoglycoside therapy, associat...

Case reports on two patients with episodic vertigo, fluctuating hearing loss and migraine responding to prophylactic drugs for migraine. Menière’s disease or migraine-associated vertigo?

Recent reports have focused on a possible association between migraine and Menière's disease; patients suffering from Menière's disease present a higher rate of migraine. In some cases, the clinical features of migraine-associated vertigo may mimic the presentation of Menière's disease. The present report focuses on two cases of females with recurrent episodes of rotational vertigo, fluctuating...
